S A N J D AR I N Environmental Health Department <br /> Y AMENDED <br /> 1/21/2020 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CITY OF RIPON WELL#18 1785 N RIPON RD, RIPON December 30, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually.This business has never made a CERS submittal for this facility. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. <br /> This violation was corrected on site. <br /> This is a Class II violation. <br /> 12 HSC 25505(c)Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. <br /> The emergency response plans and procedures,the inventory of information required by this article, and the site <br /> map required by this section shall be readily available to personnel of the business or the unified program facility <br /> with responsibilities for emergency response or training pursuant to this section. <br /> This violation was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection 12/30/2019. The report was <br /> amended by the EHD on 1/21/2020. The amended report replaces the initial report. The date of the inspection <br /> and the date of violations have been changed. The date of inspection was changed from 12/24/2019 to <br /> 12/30/2019. The violation occurred on dates were changed from 12/26/2019 to 12/30/2019. The violation comply <br /> by date was changed from 1/25/2020 to 1/29/2020. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the amended inspection report. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
