Lodi California <br /> V <br /> Client: Canadaigua/Central Cellars job #• 7-226 <br /> POTENTIAL PHYSICAL HAZARDS AT WORKSITE: Underground/overhead utility lines; <br /> fires, explosion, electrical shock, flying/failing objects; pinch points/caught between objects; <br /> exertion or strain; lifting, slipping, tripping, falling; heavy equipment and vehicle traffic at <br /> worksite; noise; burns from steam or engine parts; heat stress or exhaustion; trash with nails, <br /> broken glass,etc.;excavation collapses. <br /> CHEIINUCAL HAZARDS: Constituents of petroleum fuels,solvents,other organic compounds,in <br /> vapor, liquid or dissolved form, methane and hydrogen sulfide gas. Routes of entry include <br /> inhalation,absorbtion, contact and ingestion. <br /> RESPIRATORY PROTECTIVE EQUIPMENT: None required unless organic vapor levels exceed <br /> 10 ppm in breathing zone as measured by OVM, then half-face respirator with appropriate <br /> vapor filter cartridge as required. <br /> +-- PROTECTIVE CLOTHING AND EQUIPMENT: Level D clothing: Hardhat mandatory for all <br /> personnel working at site; steel-toed boots recommended for geologist, required for driller and <br /> helper. Ear and eye protection as needed. Chemically hazardous conditions-require nitrile <br /> gloves, Tyvek coveralls, and respirators. <br /> SITE SPECIFIC INSTRUCTIONS: Driller will examine all wires/cables and ropes daily. <br /> Drilling equipment will be maintained in safe operating condition and meet state safety <br /> requirements. Know Iocation of first aid kit, fire extinguisher, and telephone. Block/chock rig <br /> as required. No drilling or working at site without project geologist being present.
