You were <br /> notified by PHS/EHD in correspondence dated March 7, 1994 that the presence of <br /> contaminants at your facility exceed the allowable limits of leaching potential as <br /> recommended by the 1989 LUFT Manual prepared by the State Water Resources <br /> Control Board (SWRCB). At that time you were advised that PHS/EHD would be <br /> utilizing a prioritization system to evaluate your site. The results from the groundwater <br /> samples collected at the time of the August 25, 1986 tank removal and the groundwater <br /> contamination evidenced at 455 West Grant Line, indicate the possibility of a co-mingled <br /> groundwater contamination plume. Your site has been prioritized based on local <br /> groundwater conditions and placed into the Local Oversight Program for the oversight of <br /> investigation and clean up activities. <br /> The attached "Notice of Reimbursement" is official notification that PHS/EHD's <br /> Underground Storage Tank Local Oversight Program has identified you as a responsible <br /> party for the above mentioned site. Fees for review activities and inspections associated <br /> with contaminated site assessment activities will not be billed by this office. The SWRCB <br /> will invoice you directly for activities performed under the auspices of the Local <br /> Oversight Program. The goal of the site evaluation performed by PHS/EHD is to certify <br /> closures of underground tank leakage sites. Closure certification occurs only after <br /> mitigation activities are completed. <br /> In addition, the 'Regional Board Staff Recommendations For Initial Evaluation and <br /> Investigation of Underground Tanks" and "Appendix A" reporting format and the LUFT <br /> Manual shall be Utilized as guidance documents in the preparation of work plans and/or <br /> reports. <br /> N40;ision of San Joaquin Cnunty Health Care Services t <br />
