This is to notify you that Section 25297.15 (a) in <br /> Chapter 6.7 of the Health and Safety Code includes a requirement that PHSIEHD <br /> (local agency) not issue a closure letter, or make a determination that no further <br /> action is required with respect to "hazardous substance" underground storage <br /> tank release sites unless all current record owner of fee title to the site have been <br /> notified of the proposed action by the primary or active responsible party. <br /> Please provide PHSIEHD certification in writing that all record fee title owners <br /> have been notified of your request for closure. Submit a list of all the record fee <br /> title owners with their mailing addresses to PHSIEHD so they can be notified of <br /> our intent to issue a closure letter. If you are the sole property owner, provide a <br /> statement of that fact to PHSIEHD, to fulfil this requirement.
