The following is an itemized list of hazardous waste violations that have not been <br />addressed for BIG VALLEY FORD LINCOLN as of November 18, 2015. <br />Open violations from July 20, 2015 inspection <br />Violation #116 - Failed to comply with uniform hazardous waste manifest exception requirements. <br />Manifest numbers 008642979JJK and 013804208JJK were found without a signed off copy from the destination facil <br />Manifest numbers that were not found on site: <br />004164507SKS <br />004475232SKS <br />007038933JJK <br />013804898JJK <br />Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three <br />years and have them readily available for review. If the generator did not receive a copy of the manifest with the <br />handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 <br />days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or <br />the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive . <br />copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial <br />transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and <br />cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those <br />efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC for the wastE <br />oil and waste antifreeze. Submit to the EHD a copy of the signed off copy of the manifests or a copy of the exceptior <br />report submitted to DTSC. <br />Violation #301 - Facility not maintained to minimize the release of a hazardous waste. <br />An oil spill on the ground on the west side of the shop was not cleaned up . Facilities shall be maintained and <br />operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water <br />which could threaten human health or the environment. Immediately clean up the oil spill , and manage according to <br />Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste <br />was managed. <br />Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br />hazardous waste. <br />A white 5 gallon container of metal break shavings on the south side of the shop was open during time of inspection <br />All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br />close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br />Violation #501 - Failed to comply with new hazardous waste tank system assessment requirements. <br />Facility does not have a tank assessment for the 250 gallon waste oil tank and the 250 gallon waste antifreeze tank. <br />The owner or operator of a hazardous waste tank shall obtain a written assessment reviewed and certified by an <br />independent, qualified, professional engineer, registered in California attesting that the system has sufficient structur <br />integrity, is acceptable for the transferring, storing, and treating of hazardous waste, and that the tanks and <br />containment system are suitably designed. Immediately have an assessment prepared for the 250 gallon waste oil <br />tank and the 250 gallon waste antifreeze tank. Submit a schedule showing when this will be accomplished to the <br />EHD. <br />Violation #601 - Stored hazardous waste on site longer than 90 days without a permit or authorization. <br />A white 5 gallon container of metal break shavings on the south side of the shop had a start accumulation date of Ap <br />1, 2015 and on site longer than 90 days . Facilities who generate more than 1000 kg of hazardous waste per month <br />may store waste on site up to 90 days. Immediately contact a licensed hazardous waste hauler to dispose of this <br />waste under manifest and submit a copy of the manifest to the EHD. <br />Page 2 of 2 <br />
