The following is an itemized list of hazardous waste violations that have not been addressed for BIG VALLEY FORD LINCOLN as of November 18, 2015. Open violations from July 20, 2015 inspection Violation #116 - Failed to comply with uniform hazardous waste manifest exception requirements. Manifest numbers 008642979JJK and 013804208JJK were found without a signed off copy from the destination facil Manifest numbers that were not found on site: 004164507SKS 004475232SKS 007038933JJK 013804898JJK Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive . copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC for the wastE oil and waste antifreeze. Submit to the EHD a copy of the signed off copy of the manifests or a copy of the exceptior report submitted to DTSC. Violation #301 - Facility not maintained to minimize the release of a hazardous waste. An oil spill on the ground on the west side of the shop was not cleaned up . Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean up the oil spill , and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. A white 5 gallon container of metal break shavings on the south side of the shop was open during time of inspection All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. Violation #501 - Failed to comply with new hazardous waste tank system assessment requirements. Facility does not have a tank assessment for the 250 gallon waste oil tank and the 250 gallon waste antifreeze tank. The owner or operator of a hazardous waste tank shall obtain a written assessment reviewed and certified by an independent, qualified, professional engineer, registered in California attesting that the system has sufficient structur integrity, is acceptable for the transferring, storing, and treating of hazardous waste, and that the tanks and containment system are suitably designed. Immediately have an assessment prepared for the 250 gallon waste oil tank and the 250 gallon waste antifreeze tank. Submit a schedule showing when this will be accomplished to the EHD. Violation #601 - Stored hazardous waste on site longer than 90 days without a permit or authorization. A white 5 gallon container of metal break shavings on the south side of the shop had a start accumulation date of Ap 1, 2015 and on site longer than 90 days . Facilities who generate more than 1000 kg of hazardous waste per month may store waste on site up to 90 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. Page 2 of 2
