(Arcadis) is <br /> pleased to submit this Well Destruction Work Plan Addendum for the above Phone: <br /> referenced site (Site; shown on Figures 1 and 2). Twenty of the 33 Site wells 408.797.2013 <br /> have already been destroyed in accordance with the June 2, 2013 Well <br /> Destruction Work Plan submitted by Stantec Consulting Services, Inc. (Stantec Email: <br /> 2013). The Site wells were destroyed between March 23 and April 21, 2015 and Tamera.Rogers@arcadis.com <br /> documented in the Well Destruction Report submitted by Stantec on June 17, our ref: <br /> 2016 (Stantec 2016b). 60035135.1515 <br /> Due to the following health and safety or logistical impediments associated with <br /> the overdrill method, the remaining groundwater monitoring wells (VW-4 and <br /> MW-15) and sparge wells (SP-1, SP-3, SP-4, SP-5, SP-6, SP-7, SP-8, SP-9, SP- <br /> 10, SP-12, SP-13)were proposed to be destroyed by blast perforation in a Work <br /> Plan Addendum submitted by Stantec on February 19, 2016 (Stantec 2016a). <br /> • VW-4 is adjacent to identified subsurface utilities that would be damaged <br /> by over drilling; <br /> • MW-15 is adjacent to a bus stop on Pacific Avenue (a major arterial roadway), and set up of a <br /> drill rig would significantly disrupt the bus route and traffic flow; <br /> • Due to the sparge wells' diameter and depth (65 feet below ground surface[bgs]), stingers could <br /> not be used to keep the drilling augers centered on the well.
