To request a <br /> waiver, complete the enclosed "Permit Waiver Request" form and return with any additional <br /> information requested below. <br /> In order to verify your eligibility for Priority Class "B", please provide copies of the federal tax <br /> return (FTR) 1065(s) for the partnership for the years of record, 1995, 1996, and 1997. AIso, <br /> FTR 1120(s), 1041(s) or other 1065(s) as may apply for the partnership. FTR 10410(s) for the <br /> general partners of the partnership and FTR 1120(s), 1065(s), or 1041(s) as may apply for the <br /> general partners of the partnership.
