Please submit the original financial responsibility documents to the local regulatory agency and forward a copy to the Fund. Provide a copy of the most recent copy of the UST Fee Return Form filed with the State Board of Equalization (BOE) with proof of payment (copy of canceled check). The BOE letter you provided (dated May 4, 2005) is for the period January 1, 1991 through December 31, 2004. NOTE: Failure to respond to this request within thirty (30) calendar days from the date of this letter may result in an ineligibility determination of your claim.
