The addendum stated that PHS/EHD was <br /> requiring the delineation of the dissolved hydrocarbon plume. PHS/EHD stated in correspondence dated <br /> January 9, 1997 that PHS/EHD concurred with the recommendation in the "Quarterly Report- September <br /> 1996" to install two additional monitoring wells to the west and east of MW4 and MW5. When evaluating <br /> the most recent groundwater results, PHS/EHD was surprised to note that the additional well west of MW4 <br /> was not included in the addendum. The concentrations evidenced in samples collected from MW4 (13,000 <br /> ppb TPH--diesel, 190 ppb TPH-gas and 37 ppb benzene) are higher than evidenced in samples collected <br /> from MW5 (10,000 ppb TPH-diesel, 170 ppb TPH-gas and 33 ppb benzene). An additional well west of <br /> MW4 will be required to determine the lateral extent of the groundwater plume. <br /> A permit application and an $89 check was included for one additional monitoring well within the City of <br /> Tracy's right-of-way. Neither an encroachment permit nor a revocable permit was included. Please <br /> submit a revised map which includes an additional well west of MW4 and City of Tracy permits. <br /> Once PHS/EHD has issued the drilling permit, you may proceed with scheduling the field work. PHS/EHD <br /> staff must be contacted directly when field work is scheduled with the driller and at least two working <br /> days prior to initiating the field work to confirm PHS/EHD staff's schedule.
