A waste antifreeze disposal receipt <br /> dated 05/25/11 states an incorrect EPA Identification of CAL0003323356. All consolidated disposal <br /> receipt shall contain the following information: name, address, EPA identification number, contact �1 <br /> person, telephone of generator and their representative's signature, shipment date, quantity and type of <br /> waste. It also includes name, address, EPA identification of hazardous waste disposal facility where <br /> waste is to be transported along with transporter's name, address and haulers EPA identification and <br /> their signature. Failed to label universal waste to identify the type of universal waste and failed to demonstrate <br /> the length of time universal waste were accumulated. One television monitor, one computer monitor, one <br /> printer are all accumulated in room behind the service manager's office, adjacent to the service <br /> department. Per Service Director, the electronic devices were left over from previous operators. Correct <br /> by labeling the electronic devices as "Universal Waste - CRT or electronic devices". Since these it is <br /> unknown how long these electronic devices were stored onsite, immediately contact a licensed <br /> hazardous waste hauler or self-haul these electronic devices to dispose to an authorized hazardous <br /> waste disposal facility and submit a copy of the disposal receipt to the EHD. Ensure the disposal receipt <br /> remains onsite for a minimum of 3 years. <br /> 83 & 84. Failed to label universal waste to identify type of universal waste and failed to demonstrate the <br /> length of time the fluorescent tubes were accumulated. One large plastic lined box of universal waste <br /> fluorescent tubes was labeled "Universal Waste" but lacks the identification of type of waste. The <br /> fluorescent tubes were located in downstairs of Parts storage. The label listed a blank checklist of <br /> batteries, mercury thermostats, fluorescent tubes. Universal waste can be stored up to one year onsite. <br /> Per Parts manager, the box was ordered approximately 7 months ago. <br /> Immediately label the type of universal waste stored in the box. <br /> - Corrected on-site. Label was check marked "fluorescent tubes". The accumulation start date on the <br /> Universal Waste label was marked as 12/2010. Ensure disposal of universal waste fluorescent tubes are <br /> properly disposed to an authorized hazardous waste disposal facility by 12/2011 and records are kept <br /> onsite for a minimum of 3 years. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD In pe r: Received By: Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
