The contingency plan for this facility did not include correct emergency contact phone number for <br /> San Joaquin County Office of Emergency Services and it lacks contact phone number for SJC <br /> Environmental Health Department. A contingency plan shall include all of the following: <br /> -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br /> or surface water; -arrangements agreed to by local police departments, fire departments, hospitals, <br /> contractors, contact phone numbers for CAL EMA, state & local emergency response teams <br /> -names, addresses & phone numbers (office & home) of the Emergency Coordinator <br /> -list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br /> location, physical description, and a brief outline of its capabilities <br /> -evacuation plan, including routes and signals to be used. This information must be kept current at all <br /> times. Immediately update the contingency plan to include all of the necessary information. <br /> 32. A tank assessment was not available for the used oil and used antifreeze tanks. On 11/05/0, <br /> Sharon Karagozlu of KPA, stated in an email that a tank assessment inspection was completed by First <br /> Automotive and will forward assessment copies to EHD. No copy sent.This assessment shall be certified <br /> by an independent, qualified, CA professional engineer, and shall attest to the tank system's integrity. <br /> The written assessment shall determine that the tank system is adequately designed and has sufficient <br /> structural strength and compatibility with the waste stored in accordance with Title 22 regulations. <br /> Immediately obtain written tank assessments for the used oil and antifreeze tanks at this facility, <br /> maintain a copy on site at the facility, and submit a copy of each certified assessment to the EHD. <br /> Submit to SJC EHD within 30 days (07/13/11): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violations <br /> 3. requested/ supporting documentations <br /> Notes: - Facility disposal records for 07/2010 - 06/2011. Facility generated approximately 6200 gallons <br /> (24.8 Tons) of used oil, 1015 gallons (4.06 Tons) of used antifreeze, 985 gallons (3.94 Tons) of <br /> manifested hazardous waste = 32.8 Tons of hazardous waste. <br /> - Employee training records can be found in yellow Safety binder. <br /> - Need to investigate where the liquid waste from wash bay/Detail travels. There is an underground open <br /> flow-thru or tank storage? Possible oil-water separator onsite-not found. <br /> - Provided free training schedule for 2010 year.
