MARTIN LUTHER KING JR. BOULEVARD <br /> STOCKTON, CALIFORNIA <br /> CVRWQCB CASE NUMBER: 392004 <br /> CLEARTRACK ID: P-86007 <br /> Wayne Perry, Inc. (WPI), on behalf of Equilon Enterprises LLC dba Shell Oil Products US (Equilon), is <br /> submitting this well installation permit application for the former Shell service station located at 434 East Dr. <br /> Martin Luther King Jr. Boulevard in Stockton, California (Site). Two monitoring wells will be installed as <br /> requested by the Central Valley Regional Water Quality Control Board for the subject Site. <br /> The monitoring wells will be 2"diameter PVC schedule 40 wells,drilled using 8"outer diameter hollow stem <br /> augers to a total depth of 30 feet. The wells will have a 15 feet screen interval with#2/12 sand filter pack from <br /> 30 feet to two feet above the screen. Three feet of hydrated bentonite chips will be set above the filter pack, <br /> followed by neat cement to approximately 1 foot below grade. Neat cement will be mixed at a ratio of <br /> approximately 6 gallons of water to one 94-pound sack of cement and approximately 4 pounds of bentonite <br /> gel. Six inches of sand will be placed on top of the neat cement. The wells will be set in 12-inch traffic rated <br /> well boxes set in concrete to match the surface grade. <br /> Attached is the signed well installation permit application and the corresponding fees for the above referenced <br /> site.
