• Date he is to return to doctor for follow-up appointment, if necessary <br /> • Signature and address of doctor <br /> If the injured employee is unable to return to the jobsite the same day, the employee who <br /> transported him should bring this information back to the jobsite and report it to Mary Harris <br /> in Human Resources at (619) 718-9429 and the Director of Industrial Hygiene and Health & <br /> Safety, Philip Platcow at (617) 232-7355. <br /> Emergency Cases Requiring Ambulance Services <br /> ♦ Medical cases requiring ambulance services would be such cases as severe head injuries, <br /> amputations, heart attacks, etc. <br /> ♦ Should ambulance service be necessary, the following procedures should be taken <br /> immediately. <br /> • Contact necessary ambulance service and company emergency services by dialing 911 <br /> and notify the SHSO for the site. <br /> • Administer first aid until ambulance service arrives. <br /> • While the injured employee is being transported, the SHSO should contact the medical <br /> facility to be utilized. <br /> • One designated representative should accompany the injured employee to the medical <br /> facility and remain at the facility until final diagnosis and other relevant information is <br /> obtained. <br /> Death or Hospitalization of Three or More Employees <br /> The procedure as outlined in "First Aid and Medical", above, should be followed. If the injured <br /> person dies, then SECOR Human Resources Department, local officials and coroner must be <br /> notified immediately. SECOR Human Resources will notify the local OSHA office within 8 <br /> hours of the incident or fatality in the event of fatality or hospitalization of three or more <br /> employees. <br /> 5.0 Contractor Emergency Action Plan <br /> The SHSO will ensure that the Subcontractor/Contractor is capable of efficient <br /> evacuation/emergency response in the event of an emergency. Subcontractor/Contractor's <br /> employees will be trained by their employer in site-specific evacuation/emergency procedures, <br /> including alarm systems and evacuation plans and routes. <br /> The Subcontractor/Contractor shall instruct its employees that in the event of an emergency such <br /> as a fire, release, or accident involving injuries, they are required to dial 911.
