To date, <br /> you have not responded to the LOC with a request for reimbursement with eligible costs <br /> exceeding your deductible and settlement amounts (total of$241,515). If you do not <br /> submit a complete reimbursement request with eligible costs exceeding $241,515 or a <br /> written explanation as to why a reimbursement request has not been submitted since <br /> 1995 within thirty (30)calendar days from the date of this letter, we will proceed with <br /> the closure of your claim. <br /> If you are not in agreement with this decision, you may request a review of the decision by the Manager <br /> of the Underground Storage Tank Cleanup Fund Program within thirty(30)days from the date of this <br /> letter.
