(APN# 077-49-25) for the purpose of installing one groundwater monitoring well and performing <br /> quarterly groundwater monitoring and sampling activities Possible well locations are shown on the <br /> attached figure The proposed work has been requested by the San Joaquin County Environmental <br /> Health Department (EHD) <br /> Drilling should not take more than one day This work requires that up to three vehicles will remain <br /> on site during drilling The completed well will be flush with the ground surface, and will be <br /> contained within a traffic box, similar in appearance to the utility boxes already present at your site <br /> Soil and rinsate water generated during drilling activities will be stored at 1140 W Hammer Lane <br /> pending characterization and disposal <br /> Additionally, Ground Zero will require access to the property up to four times a year for an <br /> undetermined period of time to perform quarterly sampling, which is also a requirement of EHD <br /> When quarterly sampling is being conducted, a single truck will be parked next to the well for a <br /> period of approximately two hours <br /> This agreement would remain in effect for the duration of time necessary for the Trust to obtain case <br /> closure from EHD and the California Regional Water Quality Control Board (RWQCB) for the <br /> subsurface investigation associated with the property at 1140 W Hammer Lane When the Trust <br /> has obtained case closure, the well will be properly destroyed under permit from the EHD and the <br /> site will be restored to match the surrounding area Ground Zero would provide copies of our <br /> Worker's Compensation and General Liability Insurance prior to beginning any work <br /> • <br />
