AJ <br /> GROUND ZERO ANALYSIS, INC. <br /> 1714 Main Street ` !} 33 <br /> Escalon, California 95320-1927 <br /> Telephone (209)838-9888 <br /> Facsimile (209)838-9883 <br /> October 25, 2001 <br /> Mr Harlin Knoll <br /> San Joaquin County Public Health Services <br /> Environmental Health Division <br /> 304 East Weber Avenue, Third Floor <br /> Stockton, CA 95202 <br /> Subject Well Destruction Summary Report <br /> 1140 W Hammer Lane, Stockton, California <br /> Dear Mr Knoll <br /> This letter report summarizes the monitoring well destruction activities conducted at the subject <br /> site The wells were destroyed pursuant to the August 9, 2001 workplan prepared by Ground <br /> Zero Analysis, Inc (Ground Zero), which was approved by Mr Harlin Knoll of San Joaquin <br /> County Public Health Services, Environmental Health Division (PHS/EHD) The site location is <br /> shown on Figure 1 Monitoring well locations are shown on Figure 2 Monitoring well <br /> construction details are presented in Table 1 Copies of well logs are included in Attachment A <br /> Copies of the well destruction permit issued by PHS/EHD and the City of Stockton <br /> Encroachment Permit are included in Attachment B <br /> On September 25, 2001, Mr Sean Garvey of Ground Zero and Mr Knoll of PHS/EHD observed <br /> Spectrum Exploration, Inc (Spectrum, C57-512268) destroy wells MW6, MW7, and MW8 <br /> Prior to destroying the wells, depth to bottom was measured in order to ensure that no <br /> obstructions were present in the well casings Neat cement grout was then pumped down the <br /> respective well casing using a CME-55 drill rig After the casing was completely filled with grout, <br /> a pressure >_15 pounds per square inch (psi) was applied as required The grout level in the well <br /> casing was periodically checked for settling, topped off accordingly, and the pressure reapplied <br /> Approximately 20 gallons of grout was used to destroy wells MW7 and MW8 and approximately <br /> 50 gallons was used to destroy well 1\4W6 The first three feet of casing was subsequently drilled <br /> out and backfilled Backfill material in wells MW6 and MW7 consisted of approximately one foot <br /> of neat cement overlain by native soil The void in well MWS was backfilled to grade solely with <br /> neat cement at the request of the City of Stockton contractor Well destruction was inspected and <br /> approved by Mr Knoll <br /> G ID2ta1GR0UNp7,E\KN0WLES HAM\REPORTSIWELLDSTRPT doc <br />
