,a HORIZON ENVIRONMENTAL INC. <br /> Specialists in Site Assessment Remedial Testing Design and Operation <br /> f IS <br /> April 30, 2003 0 <br /> nEfg- RVEQ <br /> Mr Joe Aldridge a U N 0 2 2003 <br /> Ultramar Inc <br /> 685 West Third Street ENVIRONMENT HEALTH <br /> Hanford, California 93230 PERMIT/SERVICES <br /> Subject: Well Destruction Report <br /> Beacon Station No 3641 <br /> 1210 East Hammer Lane, Stockton, California <br /> Mr Aldridge: <br /> At the request of Ultramar Inc (Ultramar), Horizon Environmental Inc (Horizon) has <br /> prepared this report documenting the destruction of monitoring well MW-8 at the subject <br /> site located in San Joaquin County as shown on Figure 1 The San Joaquin County <br /> Environmental Health Department (SJCEHD) recommended that the well be property <br /> destroyed before being damaged during the East Hammer Lane road-widening project <br /> Horizon obtained a City of Stockton Public Works encroachment permit approved on April <br /> 7, 2003 to allow work in the median of East Hammer Lane Horizon also obtained a Well <br /> •1 Permit dated April 2, 2003 from the SJCEHD The well destruction activities were <br /> developed on the basis of the California Department of Water Resources Bulletins 74-81 <br /> (Water Well Standards) and 74-90 (California Well Standards), and SJCEHD requirements <br /> The approved permits are contained in Attachment A <br /> Well Destruction <br /> Mitchell Drilling Environmental Corporation, a C-57 licensed (#804196) drilling contractor, <br /> was retained to perform the well destruction activities Monitoring well MW-8 is located as <br /> shown on Figure 2 The well was constructed in an 8-inch diameter, 56 5-feet deep boring <br /> using 2-inch diameter PVC well casing and screen (35 to 55 feet bsg) Prior to initiating <br /> well destruction activities the total depth of the well casing was measured to confirm that <br /> there were no obstructions The measured depth of monitoring well MW-8 casing was 54 5 <br /> feet <br /> Well destruction activities included the following tasks <br /> 1 43 sand was placed in the well casing from total depth to 40 feet bsg, <br /> 2 pilot rod was lowered into the well casing to the level of the sand, <br /> 3 the well boring was drilled out to 60 feet using 8-inch diameter hollow-stem augers, <br /> 4 the pilot rod and the well casing were extracted from the hollow stem and the auger bit <br /> • was pulled back approximately 3 feet, <br /> 5011 Golden Foothill Parkway Suite 7 • EI Dorado Hills CA 95762 • (916) 939-2170 • FAX (916) 939-2172 <br /> PO Box 5283 • Bakersfield CA 93388 9 (661) 589-8389 • FAX (661) 589-1456 <br />
