A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer <br /> drains, access and exitpoints, emergency shutoffs, evacuation staging areas, hazardous material handling and <br /> storage areas, and emergency response equipment. If a site map element is not applicable for your facility <br /> then list it on the map and label as "NA". <br /> • Violation#7, Emergency Response/Contingency Plan is not complete. <br /> - The plans were lacking procedures for the immediate notification the Environmental Health <br /> Department (EHD) at 209-468-3420 and the California State Warning Center/Cal OES at 1-800-852- <br /> 7550. <br /> - Note,the Consolidated Emergency Response/Contingency Plan Template may be used to satisfy <br /> requirements that Hazardous Materials Business Plans (HMBP) contain emergency response plans, <br /> procedures, and employee training in the event of a reportable/threatened hazardous material release <br /> (see attached). <br /> • Violation#8, Employee Training Plan is not adequate. <br /> - The training plan does not address the frequency of training, such as training for all new <br /> employees and annual training, including refresher courses,for all employees in safety procedures in <br /> the event of a release or threatened release of a hazardous material, including, but not limited to: <br /> ■ Familiarity with the emergency response plans and procedures in the event of a release or <br /> threatened release of a hazardous material. <br /> ■ Immediate notification contacts to the appropriate local emergency response personnel and <br /> to the unified program agency. <br /> ■ Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property, or the environment. <br /> ■ Note,the Consolidated Emergency Response/Contingency Plan Template may be used to <br /> satisfy requirements that Hazardous Materials Business Plans (HMBP) contain emergency <br /> response plans, procedures, and employee training in the event of a reportable/threatened <br /> hazardous material release (see attached). <br />
