• Violation#7, Emergency Response/Contingency Plan is not complete (see attached Emergency <br /> Response/Contingency Plan Template). <br /> 1. The Emergency Assembly Area(s) have not been identified. <br /> 2. The Emergency Response Equipment type and locations have not been addressed. <br /> - Note,the Consolidated Emergency Response/Contingency Plan Template may be used to satisfy <br /> requirements that Hazardous Materials Business Plans (HMBP) contain emergency response plans, <br /> procedures, and employee training in the event of a reportable/threatened hazardous material release <br /> (see attached). <br /> • Violation#8 has been closed. <br /> • Violation#11—Records showing the employees have received training in the following elements have not been <br /> received (do not upload employee training records to CERS). <br /> 1. Familiarity with the emergency response plans and procedures in the event of a release or <br /> threatened release of a hazardous material; (Emergency Response/Contingency Plan familiarization). <br /> 2. Immediate notification contacts to the appropriate local emergency response personnel and to the <br /> unified program agency; (spill reporting). <br /> 3. Procedures for the mitigation of a release or threatened release to minimize any potential harm or <br /> damage to persons, property, or the environment; (spill response). <br /> 4. Evacuation plans and procedures, including immediate notice,for the business site; (evacuation <br /> training). <br /> • Violations 2, 3, 3, 7, 9, 11,and 12 remain open. Violations 2, 3,9, and 12 will remain open until violation 5 and 7 <br /> have been successfully addressed. <br /> Be advised, the violation comply by date of November 15, 2019 has passed, consequently all compliance <br /> activity is be billed at the EHD hourly rate ($152). <br /> 2 <br />
