The soil samples had no detections above the laboratory <br /> method reporting limits. The site has been sampled quarterly since February 1995. <br /> PROPOSED SCOPE OF WORK <br /> The proposed scope of work for this work plan includes destroying one groundwater monitoring well <br /> (MW-3) . Details for the activities associated with the completion of the scope of work are provided <br /> below. <br /> Site Health and Safety Plan <br /> As required by the Occupational Health and Safety Administration (OSHA) "Hazardous Waste <br /> Operations and Emergency Response' guidelines (29 CFR 1910. 120) and by California Occupational <br /> Health and Safety Administration (Cal-OSHA) "Hazardous Waste Operations and Emergency <br /> Response" guidelines (CCR Title 8, Section 5192), the contractor will prepare a site specific Health <br /> and Safety Plan (HASP) . At a minimum , the HASP will define the proposed subsurface investigation <br /> activities, describe physical and chemical hazards that may be associated with the work, provide a <br /> map to the nearest emergency medical facility, and include material safety data sheets for any <br /> hazardous chemicals that will be used or produced during the work. A copy of the HASP will be <br /> available onsite at all times during field work. The field staff and contractors performing field activities <br /> will review the HASP prior to beginning field operations, as described in this work plan , at the site. <br /> Permitting <br /> Prior to beginning field work, all necessary permits will be obtained . <br /> Well Destruction <br /> The destruction of well MW-3 will be supervised by Stratus. A licensed drilling company will <br /> complete the well destruction work following the California Well Standards bulletin 74-90 Part III , <br /> Section 19 and 23 and San Joaquin County Well Standards Section 13. 17 . <br /> MW-3 will be destroyed by removing all of the well contents via over-drilling the borehole to a total <br /> depth of 63 ft, then backfilling the borehole to three ft bgs with neat cement using a tremie pipe <br /> system forming a mushroom grout cap/surface seal . Groundwater expelled from the well during the <br /> grouting process will be properly contained , characterized , and disposed of. After the sealing <br /> operation , the borehole shall be backfilled with clean , native soil . The former well will be capped <br /> with appropriate materials to match existing grade. Well construction details can be found in <br /> Table 1 . The boring log for MW-3 is presented in Attachment B. <br /> REPORTING <br /> A letter summarizing the well destruction and soil disposal activities will be prepared and submitted <br /> to the California Regional Water Quality Control Board and the SJC/EHD. The letter will include <br /> copies of the well destruction permit. In addition , this report will be uploaded into Geotracker. <br /> SCHEDULE <br /> SECOR is prepared to initiate field activities upon approval of this work plan by the SJC/EHD . <br /> SECOR anticipates that the drilling portion of the fieldwork will require approximately four to six <br /> 548 Work Plan for Well Oestruction.doc SECOR Intemationa/ Incorporated <br />
