A more'',detailed summary of the incident is
included as Attachment A. Our staff was not injured during the incident; however, based upon
the findings during the accident review, Stantec and ARC}havel, rdecided to eliminate and/or
reduce the sample frequency for wells in or near roadways.
F .E
There are currently two high-risk well locations at the site (Attachment B).- These wells include
MW-6 and MW-7 (both of which are located on Brentwood Drive), "which are currently sampled
on a quarterly basis. A review of historical data (Attachment C) shows that groundwater
analytical concentrations have been low to non-detect since February 15, 1995 in MW-6 and
since December 13, 2004 in MW-7. In addition to the low to non-detect concentrations, active
air sparging has been performed at the site since August 24, 2007. Based on the low to non-
detect petroleum hydrocarbon concentrations, remedial activities, and the over-riding risk to
worker safety, it has been determined that MW-6 and MVI!-7 will be sampled annually (first
quarter) rather than quarterly.
548 Sampling Reduction Notification.doc „ 1
