Supplemental Investigation Report <br /> BP Oil Site No 11193 <br /> C A !1A B R I A Stockton, California <br /> October 13, 2000 <br /> WELL INSTALLATION ACTIVITIES <br /> Wells MW-1 and MW-2 were replaced with 4-inch diameter wells,and one additional 4-mch well and <br /> two 2-inch wells were installed for lateral and vertical definition Cambria's standard procedures for <br /> soil sampling and well installation and for abandoning monitoring wells are attached as Appendix B <br /> Personnel Present Jacquelyn Jones, Cambria Geologist, working under the supervision of <br /> Khaled Rahman, California Registered Geologist Carol Oz and <br /> Margaret Lagono of the SJCEHD were onsite intermittently to inspect <br /> field activities <br /> WIffirm <br /> NNW <br /> Number of Wells Two replacement wells (MW-1R and MW-2R) and three new wells <br /> (MW-8, MW-9 and MW-10) <br /> Well Locations Well MW-1R was installed adjacent to destroyed well MW-1 Well <br /> MW-2 was over-drilled, and well MW-2R was installed in the same <br /> • boring Well MW-8 was installed onsite approximately 20 feet northeast <br /> of the underground storage tanks (USTs) Well MW-9 was installed <br /> onsite approximately 35 feet west of the pump islands Well MW-10 was <br /> installed offsite between the car wash bays and the vacuum bays of the <br /> adjacent property, the Kwik Kar Wash (see Figure 2) <br /> Permits SJCEHD Dnlling Permit Nos 0022815 and 0022816 were issued for the <br /> installation of onsite monitoring wells MW-1R, MW-2R, MW-8 and <br /> MW-9, and for the destruction of well MW-2, respectively SJCEHD <br /> Drilling Permit No 0022814 was issued for installation of offsite well <br /> MW-10 (see Appendix C) <br /> Dnlling Company V&W Drilling(V&W)of Rio Vista, California(C-57 License#720904) <br /> Drilling Date June 12 and 13, 2000 <br /> 6 <br />
