(ATC) for the above,-referenced site on your behalf. The work plan <br /> was submitted in response to an EHD letter dated September 24, 2009, concerning the <br /> locations for advancing soil boring SB-4 and installing monitoring well MW-11, and the well <br /> construction of MW-11. <br /> ATC has illustrated the proposed locations for SB-4 and MW-11 in Figure 1 of the second <br /> addendum work plan. The EHD approves the relocation of SB-4 approximately thirty feet <br /> southeast of monitoring well MW-3, and MW-11 adjacent to soil boring ATC-5. <br /> Although ATC proposes to install MW-11 using a combination drill rig equipped with eight- <br /> inch-outer-diameter hollow stem augers and direct push technology, there is no mention of <br /> advancing the borehole using the eight-inch-outer-diameter hollow stem augers. ATC <br /> states that a nominal one-and-a-half-inch diameter borehole will be advanced to a depth of <br /> approximately twenty-five feet below surface grade while collecting soil samples at five- <br /> foot intervals for lithologic logging and field screening. The work plan states that upon <br /> completion of the soil boring, MW-11 will be constructed with a five-foot screen of 0.020- <br /> inch slotted Schedule 40 two-inch diameter polyvinyl chloride. One must assume that <br /> upon completion of the advancement of the initial one-and-a-half-inch soil boring and prior <br /> to constructing the well, the borehole will be over-drilled using eight-inch-outer-diameter <br /> hollow stem augers. Also, the EHD recommends you use Number 3 sand instead of 2/20 <br /> Monterey sand for the sand pack since the well screen consists of 0.020-inch slotted <br /> Schedule 40 casing. <br /> Please submit a boring permit application and permit/inspection fee of $434 to the EHD <br /> and complete the approved fieldwork by February 1, 2010. Notify the EHD of pending <br /> fieldwork at least a week prior to the scheduled date. <br />
