PHS/EHD has also evaluated documents in our files for <br /> the above referenced site, and has the following comments'. , <br /> I <br /> F <br /> 1. PHS/EHD was not notified of the October 23 ,ij 1991 sampling <br /> event. Please be informed that PHS/EHDinspection of sampling <br /> events are an important aspect of local regularory oversight. <br /> Prior to any future sampling event, please contact Mary Meays, <br /> at the number below, 48 hours in advance. <br /> 2 . The PHS/EHD letter dated October 17, . 31991 requested thatila <br /> work plan to define the plume of groundwater contamination be <br /> submitted within 30 days. The work plan requested , has not <br /> been submitted to date. The additional investigation is <br /> essential to evaluate the impact to, groundwater quality. <br /> Submit a work plan for the additional soil/groundwater <br /> investigation by January 31, 1992 . <br /> 3 . The November Vapor Extraction Report iridicatedllthat the system <br /> was in operation for only one day ini November 1991. When <br /> vapor extraction began on November 16, ;1990, the daily average <br /> influent concentration was 6, 533 ppm TIPH. The current daily <br /> average influent concentration, November 20, 1991, was 205 ppm <br /> TPH. <br /> According to the March 25, 1991 Installation, Start=up and <br /> Operation of the Vapor Extraction and Treatment System; (VETS) <br /> Report prepared by EA, "After they major ° mass ' of the <br /> hydrocarbons has been removed by these .threelwells (V3, V5, <br /> and MW4) the other six wells (V1, V2, iV4, MW31, MW5, and MW6) <br /> will be used to extract vapors.
