Soil and groundwater contamination was indicated by the sample results from the July 16,1991 soil <br /> boring, which was converted to monitoring well MW-9, and by the July 24, 1991 groundwater <br /> sample from MW-9. <br /> 2. Comparison of soil results from MW-9 and vapor extraction well V-3 does show that there has been <br /> some reduction of hydrocarbon contamination. During the September 10, 1991, PHS/EHD phone <br /> conversation with Clint Rogers,you stated that there were no immediate plans to shut-off the vapor <br /> extraction system. Further confirmatory soil sampling will be necessary to evaluate the effectiveness <br /> of the vapor extraction system. <br /> 3. Commence quarterly groundwater monitoring. Quarterly reports are recognized as a minimum <br /> reporting requirement under Title 23 of California Code of Regulations, Chapter 16, Article 5, <br /> Section 2652. Quarterly monitoring reports shall be submitted to Public Health Services, <br /> Environmental Health Division and Central Valley Regional Water Quality Control Board <br /> (CVRWQCB) until the investigation and cleanup of the site is deemed adequate. The report is to <br /> show the: <br /> -status of investigation and cleanup activities including the results of all investigations <br /> completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion of the <br /> investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and any contaminated <br /> soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if known, and changes in the above <br /> measurements over the last quarter. <br /> Submit the next quarterly report within 30 days of the date of this letter. <br /> A Division of San Joaquin Gmnty Health Caet,"ices 10 <br />
