A well/sensitive receptor survey should be performed to identify potential sensitive receptors <br /> within a 2,000-foot radius of the investigation area. <br /> 10. Preparation of a technical report. The report should include an analysis of data obtained <br /> during the investigation and at least two cross-sectional views of site lithology and <br /> hydrocarbon-impaction.Recommendations for additional investigation or site closure should <br /> be included. The report should be reviewed and signed by a California Registered Geologist. <br /> 11. Regulatory liaison. <br /> The attached bid form should be completed and accompany all submitted bids. The following <br /> additional requirements will apply: <br /> 1. The consultant must carry out the described work in a timely,professional, safe and legal <br /> manner and maintain necessary records and other safeguards to ensure that reported sample <br /> results are accurate. All work must be performed in a manner acceptable to the PHS-EHD. <br /> 2. Consultant shall at its own cost and expense provide and maintain workers compensation <br /> insurance meeting all requirements of the State of California <br /> 3. Consultant shall provide and maintain general liability insurance with an A+rated carrier <br /> admitted to do business in California in the minimum amount of 1 million dollars per <br /> occurrence with a minimum total limit of 1 million dollars. Prior to start of work,client must <br /> be named as an additional insured on said general liability insurance. A certificate of <br /> insurance of both the general liability insurance and workers compensation insurance must <br /> be provided to client prior to start of work.All insurance must be maintained in full force and <br /> effect during the course of the contract. Client shall not be responsible for the cost of any <br /> associated premiums. Consultant shall be responsible for any third party claims arising out <br /> of its work. <br /> 4. Work shall be performed in a manner which will not require the cessation of business at the <br /> site unless absolutely necessary or dictated by the PHS-EHD. <br /> 5. Consultant shall provide a copy of consultant's standard service agreement and schedule of <br /> charges with any bid submitted. The schedule of charges shall include: <br /> a. Hourly rates for personnel classifications cnrployed by the consultant. <br /> b. Unit charges for common supplies and equipment used in corrective action work. <br /> C. Multipliers used when charging any other direct expenses(i.e. subcontractors). <br /> 6. Consultant understands that client intends to seek reimbursement for the subject work from <br /> the UST Cleanup Fund administered by the State Water Resources Control Board,and will <br /> agree to respond to any reasonable inquiry from that agency regarding any claim submitted <br />
