Box 1000 <br /> Lodi , CA 95241-1000 <br /> RE: MS-87-49 <br /> This application was approved by the Planning Division for the above referenced <br /> premise. The conditions imposed by the Local Health District were made a part <br /> of the approval : <br /> 1. This tentative division of land shall meet the Rules and Regulations of <br /> San Joaquin County Ordinance No. 549. A percolation test and a minimum <br /> sewage disposal area are required for Local Health District approval . A <br /> permit for the percolation test from the Health District is required. <br /> 2. Provide an "Improvement Plan" showing the location of the well and sewage <br /> disposal area prior to the recording of the final map. <br /> Since approval of this permit included San Joaquin Local Health District conditions , <br /> you are hereby advised that the conditions shall be met prior to utilization of the <br /> approved land use. <br /> If the decision is appealed to the Board of Supervisors , the filing of the appeal stays <br /> the issuance of this permit or any other permits until the appeal has been decided by <br /> the Board. <br /> If you have any questions regarding this matter, call (209) 466-6781. <br /> Jogi Khanna , M.D. , M.P.H. <br /> District Heal h Officer <br /> Ron L.
