(AGE) has <br /> prepared this work plan for the destruction of ground water monitoring well MW-I 1 at 375 West <br /> Hazelton Avenue, Stockton, California (site). The site and surrounding area are illustrated on <br /> Figure 1; structures, soil borings, and monitoring well locations are illustrated on Figure 2. Also <br /> enclosed with this work plan is a well destruction permit and the associated well destruction permit <br /> fee. <br /> INTRODUCTIONBACKGROUND <br /> To date,a total of six ground water monitoring wells have been installed on-site.On 29 March 2005, <br /> three ground water monitoring wells(MW-9 through MW-11)were installed at the site using 2-inch <br /> diameter, Schedule 40 polyvinylchloride (PVC) casing with a 0.020-inch slotted screen installed <br /> from 20 feet to 40 feet below surface grade (bsg). On 30 and 31 January 2006, three additional <br /> ground water monitoring wells (MW-12 through MW-14) were installed at the site using 2-inch <br /> diameter, Schedule 40 PVC casing with a 0.020-inch slotted screen installed from 20 feet to 40 feet <br /> bsg in well MW-12 and from 55 feet to 65 feet in wells MW-13 and MW-14. <br /> It has recently been determined that an on-site sewer line, located approximately 10 feet bsg, was <br /> damaged during the installation of well MW-11 in March 2005. In order to repair the sewer line, it <br /> must be excavated and exposed, and the integrity of well MW-I 1 will be compromised.
