Box 60, Tracy, California <br /> Dear Mr. Knoll: <br /> Ground Zero submitted the Workplan for Well Destruction (Workplan) on June 23, 2011. In a <br /> letter received on July 11, 2011, the San Joaquin County Environmental Health Department <br /> (EHD) approved the Workplan. A permit package and associated fees was submitted to EHD and <br /> the well destruction permit number SR#62989 was issued on July 14, 2011. <br /> The location of the site is shown on Figure 1 and the location of the destroyed well was northeast <br /> of the former UST B as shown on Figure 2. <br /> Well Destruction <br /> On July 19, 2011 well destruction activities were coordinated with Mr. Harlin Knolls of EHD. <br /> On July 20, 2011, V&W Drilling (C57 #720904) destroyed monitoring well MW-BR under the <br /> supervision of Mr. Joe Vasquez, a Ground Zero Geologist. <br /> The depth to bottom of the monitoring well was measured to insure no obstructions were present. <br /> The well was over-drilled using 8-inch diameter hollow stem augers center overthe well casing <br /> using a guide rod. After the well was over-drilled the guide rod was removed`and the 2-inch- <br /> diameter <br /> -inchdiameter well casing pulled from the borehole. The boring was grouted with neat cement gout, <br /> through the augers grade level. No water was displaced during well destruction activities. Surface <br /> completion consisted of native fill to match the existing surface.
