Main St. <br /> Stockton, CA 95202 <br /> Subject: Well Destruction Permits for <br /> San Joaquin Catholic Cemetery <br /> Dear Mr. Knoll: <br /> Enclosed is a check in the amount of$354.00 for permit and inspection fees as <br /> requested in your August 30, 2007 FAX. Also enclosed is a completed well destruction <br /> permit application, a completed Licensed Contractors Declaration (LCD), a map <br /> showing the location of the wells, and a list of wells to be abandoned showing diameter <br /> and total depth. <br /> Doulos Environmental, Inc. (Doulos) will abandon these wells by pressure grouting the <br /> wells. Prior to field work, calculations will be made to determine the amount of grout <br /> necessary to fill the casing and gravel pack surrounding the well. Portland cement will <br /> be trimmied from the bottom to the surface of the well. Then, approximately 25 PSI of <br /> air pressure will be applied to the well for approximately 5 minutes. If necessary <br /> additional grout will be added after pressure is applied. The total amount of grout used <br /> for each well will be recorded and compared with the calculated amount to ensure <br /> sufficient grout has been used to fill the casing and the pore space surrounding the well. <br /> After all wells have been pressure grouted, the upper portion of each well will be <br /> removed. More grout will be added to mushroom over the edge of the casing. <br /> Doulos would like to begin this work as soon as possible.
