Also detailed in this correspondence was a request to enact an approved work plan from <br /> November 1997',and the work plan addendum approved on November 4, 1998. As of the date of this correspondence, <br /> the work plan has not been enacted nor has the requested information been received at this office. <br /> In addition,PHS-EHD has not received a quarterly monitoring report for the three (3) monitoring wells located on site <br /> since-the sampling event held on November 18, 1998. <br /> PHS-EHD has been informed by the State Water Resources Control Board(SWRCB)Clean Up Fund(CUF)that you <br /> have been reimbursed for previous investigation costs. Sites are required to be in compliance with directives and { <br /> requirements issued by PHS-EHD in order to continue to receive investigation reimbursement costs. <br /> You are hereby,directed to submit the underground storage tank piping information that supports conclusions <br /> that the proper piping destruction/removal and required soil sampling has occurred. Additionally,you are <br /> directed to resume Quarterly Monitoring of the three monitoring wells on site immediately and submit the <br /> report of findings before the next sampling event. The Quarterly Monitoring Report for the first quarter-2000 <br /> (January,February,March)is due at PHS-EHD before April 17,2000. <br /> Additionally,you are hereby directed to submit a boring permit application and$89 fee by February 10,2000 <br /> li and enact the approved work plan and addendum before March 10,2000. <br /> If you are unable to provide documentation for the proper piping destruction and the related soil sampling,PHS-EHD <br /> recommends your work plan be expanded to include this requirement. Closure documents can not be issued until the <br /> underground storage tank closure (includes piping) is complete.
