Minimum information requirements are listed on page two of the appendix. In i <br /> order to properly evaluate this underground storage tank (UST) site for closure, additional <br />' information is needed. Please ensure chat the minimum requirements listed on page two of <br /> Appendix B are included in any Closure Summary Report submitted to PHS-EHD for review. <br /> Table 4 of the closure report listed some of the groundwater samples in the BTEX columns as <br /> %5.0" A footnote advised that the detection limit units were in uyfl=mkn ream per ii erorpzm per <br /> bd1kr4 ppb". The laboratory data sheet for these groundwater samples indicated that the detection <br /> limit was 0.5 ug/l. Table 4 should be corrected to reflect the actual laboratory detection limits. <br /> Included in the PIER submitted by AGE on October 25, 1996 are the boring logs for soil borings <br /> B-1 through B-4 from July 1996 field work. Notations indicated that strongHCodor"was detected <br /> in B-1, B-2, and B-4 at 7 and 10 feet below ground surface (` bgs). Unfortunately,these soil samples <br /> were not the soil samples that were analyzed at the laboratory. Confirmation soil samples are to be <br /> collected from all areas that indicated "stmTHCalon"were present before evaluating a closure ' <br /> report. <br /> In addition,it is unclear whether the piping was removed at the time the USTs were removed. No <br /> "piping" soil samples are included in any report and transport manifests do not indicate that any <br /> length of piping material was transported offsite. Verification of the piping removal and proper soil <br /> sampling is required before closure can be approved. If verification attempts indicate that the piping <br /> is still present at the site, soil samples are to be collected and analyzed from all piping areas and the <br /> piping is to be'removed or destroyed (grouted) in place under a permit issued by PHS-EHD.
