ECM will be the third party billing agent.At the <br /> bottom of the MFR form, Binayak Acharya's signature is required as representative of ECM and as the <br /> permit applicant. The form also requires Binayak's Driver License Number and a photocopy of his license. <br /> I do not think this is appropriate since Binayak is not personally responsible for the costs- ECM is. Is <br /> there something else more appropriate I could put instead, such as ECM's tax ID number?
