M <br /> Physical Hazards: These hazards are primarily associated with onsite equipment and the <br /> general nature of construction work. Please refer to the attached Physical Hazard Analysis <br /> Sheets for controls. <br /> Heat X Slip, Trip, Fall Excavations/Trenches <br /> Cold X Noise X Moving Equipment <br /> Rain X Underground Hazards X Traffic <br /> Fog Overhead Hazards Other: <br /> =Level Protection: ❑ A ❑ B ❑ C ■ D <br /> Modifications: Modifications to levels of protection shall be made by the SSO based on <br /> air monitoring results. <br /> Site Entry Procedures: The site will be entered from North Hunter Street. <br /> Site Control Measures : Unauthorized personnel will not be allowed in the work area. No <br /> eating, drinking or smoking in the work area. The Field Supervisor shall enforce these measures <br /> and buddy system, PPE and training requirements for all on site personnel. Spills are not <br /> expected to be a concern on the project. However, if a spill is to occur, adsorbent materials are <br /> available. <br /> A tailgate safety meeting will be held by the SSO for all site personnel. The safety discussion <br /> will include, in addition to other items in this Health and Safety Plan, the following issues: <br /> • Site personnel will execute proper lifting techniques for heavy items. The <br /> maximum weight to be lifted by any site personnel should not exceed 60 lbs. <br /> • The drill rig shall be inspected prior to operation by the drill crew and SSO. <br /> • An exclusion zone shall be secured around the work area. <br /> • Fire extinguisher, first aid kit, eye wash, and hospital route map shall be staged for <br /> easy access during drilling activities. <br /> • Onsite personnel shall contact the SSO with any Health and Safety issues. <br /> 4 <br />
