x a CONTRACT FOR J ANDFILL SERVICES This Contract is entered into by and between Forward Landfill and Ground Zero Analysis. Ground ZeroAnalysis desires to contract for landfill services to dispose of Septic Tank Cleanout Waste from a property at 5781 - $787 Jahant Rd., ACAMPO. Forward Landfill Ctasires to provide to Ground Zero Analysis landfill services. In considamtion of the following mutual covanants, the part;es agree as follows: "., Fc ,srd Landfill agrees to provide landf=ill disposal services for Septic Tank Cleanout Waste from the property of 6781 -6787 Jahant Rd.,ACAMPO only. Forward Landfill is not responsible for providing any services other than those specified herein. 2 GroGnd Zero Analysis agrees to compensate Forward Landfill for the landfii; services as follows: Ground Zero.Analysis shall pay a disposal fee of $45.00 per drum. All of the foregoing compensation is considered due immediately upon acceptance by Forward Landfill of waste material for transport and/or storage. On any balance that is 60 or more days overdue, interest shall aocrue zt monthly rate of two percent(20/D) until paid in ful'. 1f at any firna the balance owing to FOP%vara Landfill excseds MOM or if at anytime any amounts are 30 o--ore days overdue , Forward Landfill shail have the right to suspend its further performance under this agraoment until paym-n:in full is received. 71�i$ agreement shall not be binding on Forward Landfill and no waste will be accepted by Forward Landfill u^der this agreement unt;l a signed copy of this agreement has been faxed to Forward Landfill at (209)466- 1057, or by sv&other means as Forward Landfill may hereafter specify in,writing, if custorner does not provide a signed cozy of this agreement to Forward Landfill within 15 days of the agreement being signed by Forward Landfill• the offer of Forward Landfill to enter into this agreements' be deemed reevoked. This agrebrrre�nt shall remain in effect until May 5, 2003. The termination of this agreement sha',l not affect the oblige#ion of Forward Lartufill or the personal gi,iaiantor to pay amounts due under this agreement. 4. =_a:h load of waste materiai must be accompanied by a manifest that specifically identiffies the property from which the waste material derives. The approval number assigned to Ground Zero Analysis for the project shall be used by Ground Zero Analysis on ail documentation and correspondence. The approval number assigned to ;-ourd Zero Analysis for this project is: 2597. Forward Landfill shall have the right, in its sole discretion,to refect any loads not complying with the documentation requirements. However,acceptance by Landfill of a load of material without compliant documentation shill not constitute a waiver by Forward Landfill of any rights under this agreement. 5. Ground Zero Analysis represents and warrants that it will comply with all sta a and federal statutory and regulatory requirements aQpllcable to the transportation and disposal of the waste in question, and that only waste from the Specified property will be provided to Landfill for disposal.
