' After the grout has cured overnight, the well head will be finished in a manner <br /> substantially identical to existing well MW3. The well head will consist of a flush mount <br /> well-head vault set in a 2-foot square concrete pad at the surface and the pad will be <br /> ' finished to slope away from the center. The well head will be secured with a water tight <br /> well head cap and a padlock. A reference mark will be placed on top of the well casing, <br /> and the horizontal location and elevation of the reference mark and ground level will be <br /> ' determined by professional survey. It is anticipated that the reference elevation for the <br /> professional survey will be NGVD 29 and the horizontal coordinates will be based on the <br /> California Coordinate System 83. <br /> Documentation and Reporting <br /> ' A field geologist under the supervision of a California Professional Geologist shall be on- <br /> site during all drilling and well construction activities in order to log and describe soil <br /> samples consistent with the Unified Soils Classification System, observe drilling <br /> ' procedures,watch for indications of groundwater during drilling, direct well construction <br /> activities, record materials used, and act as safety officer. <br /> ' The field geologist will create a well completion and boring log for each well or boring. <br /> The log will document the as-built well construction features and the subsurface <br /> conditions encountered during drilling. A report of activities will be compiled based <br /> largely upon the on-site geologist's boring logs and the surveyor's report, and it shall be <br /> submitted to Board Staff. The report shall include, but not be limited to,boring logs and <br /> well construction logs for B5, B6, and B7/MW5, results of laboratory analysis of the grab <br /> ' groundwater sample at MW5, a log documenting well abandonment of MW3, and results <br /> of professional survey at MW5. No deviations from this work plan are anticipated. <br /> However, should field conditions warrant modification of well construction or sampling <br /> ' plans identified in this document, then the deviations shall be documented in the report of <br /> activities. <br /> The proposed schedule for this work is that it is to be completed within 60 days after <br /> receiving approval of the workplan. If approval of the plan is received prior to July 1, <br /> 2005, then the Well Installation Report will be completed by August 31, 2005. <br /> June 21,2005 Page 5 <br />
