The RWD describes the anticipated discharge <br /> of brine concentrate generated from a reverse osmosis treatment plant. The brine concentrate will be <br /> solar dried in the evaporation ponds and hauled to a landfill. The RWD is incomplete for the following <br /> reasons: <br /> 1. The RWD needs to be submitted under the signature and stamp of a California Registered <br /> Engineer or California Certified Engineering Geologist. <br /> 2. The initial review of the RWD results in a tentative rating of"213"based on the threat to water <br /> quality and complexity. The Annual Fee Schedule has been enclosed for your use. With a rating <br /> of"213", the annual fee will be $9,000. This amount should be made payable to the State Water <br /> Resources Control Board and submitted to the Central Valley Regional Water Quality Control <br /> Board at this time. <br /> 3. Form 200 Standard Application: The Standard Form 200 needs to be revised. The instructions <br /> are enclosed to provide additional clarification to these comments. The items that need to be <br /> furnished are as follows: <br /> a. I. FACILITY INFORMATION: Need more information regarding the Facility Owner, <br /> Facility Operator and Land Owner. The government agency that owns the facility needs <br /> to be named (e.g. California Department of Corrections). A representative of that agency <br /> shall also be provided and that person should also sign the certification statement of the <br /> Form 200. <br /> b. III. LOCATION OF FACILITY: The Assessor Parcel Numbers, Longitude and Latitude <br /> must be included. <br /> c. VIII. CERTIFICATION: The Correctional Plant Manager is not considered an <br /> appropriate representative to sign the certification statement. The instructions provide <br /> information as to whom can sign this application form, and for a governmental or public <br /> California Environmental Protection Agency <br /> 0 Recycled Paper <br />
