The permit has <br /> been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at(209)953-7699 to arrange the following appointments(a 48-hour notice is <br /> required for inspection appointments). <br /> • Witness soil sampling below removed UDC and piping and receive laboratory analytical results; receive all <br /> appropriate waste disposal documentation for removed piping and sump(s) <br /> • Primary piping soap test: One hour pneumatic pressure test for all repared regulated piping, at 110%of <br /> operating pressure, or 40 psi,whichever is greater. <br /> Secondary piping soap test, piping slope(1/8 inches per foot), and piping set backs(per manufacturer's <br /> installation instructions)of all piping prior to backfill: One hour pressure soap test for all piping, per <br /> manufacturer's guidelines. If there are no manufacturer's guidelines,tests shall be performed using an <br /> applicable methods specified in an industry code or engineering standard. <br /> • Verify UDC installation,verify UDC pretests prior to backfill, and verify correct brine addition, per the manufacturer's <br /> installation instructions. <br /> • Verify 91 STP sump repairs, all 91 STP sump pretests, and verify correct brine addition, per the manufacturer's installation <br /> instructions. <br /> Sensor functionality and brine/vacuum communications for UDC 5/6, 91 STP sump, all <br /> impacted piping (product and vent), and 91 tank, in operating condition: Functionality of the <br /> monitoring system must be verified to ensure that monitoring has been restored to meet the <br /> requirements of Title 23 California Code of Regulations and the Health&Safety Code. If it is <br /> determined that a cold start is necessary, a copy of the alarm history shall be printed prior to <br /> performing the"cold start"of the monitoring system and made available at the time of EHD <br /> inspection. <br /> • Verification of the dispenser footprint contained by the UDC 5/6 and visual piping and fule filter inspection within the UDC <br /> Please be aware that the facility cannot operate the UST system prior to the EHD inspection <br /> and approval. <br /> 1868 E.
