The permit has been approved with the <br /> 7 follo ' g conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elianna Florido at (209) 468-0343 to arrange the following appointments (a 48-hour <br /> notice is required for inspection appointments). Please be aware that inspection for emergency <br /> work must be scheduled within five days of permit approval. <br /> • Verify that the Bravo test ports were installed according to manufacturer's instructions. <br /> • Verify that the correct brine is placed into the interstitial space of UDC 5/6. <br /> • Verify brine communication in UDC 5/6. <br /> • Ensure sensor functionality in UDC 5/6. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be performed on <br /> the affected USTs once concrete is poured in accordance with the manufacture's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> 1868 E.
