The interstitial space of UDC <br /> 5/6 shall be maintained as designed under constant vacuum such that a breach in the primary or secondary <br /> containment is detected before the hazardous substance is released into the environment. Immediately contact a <br /> properly licensed, trained, and certified contractor to repair communication in UDC 5/6. Obtain a permit if needed. <br /> Notify the EHD of the scope of work and time and date of the repair by emailing UST@sjcehd.com. The service <br /> company must install the part and test the system to verify functionality. The service company must provide the <br /> following documentation to the tank owner/operator for inspector review during annual UST inspection: a) <br /> description of part(s) installed, b) name & UST ICC#of service technician doing the work (manufacturer certification <br /> proof), c)testing done to verify functionality of the repair. <br /> This is a repeat violation, Class ll. <br /> 316 HSC 25290.1(c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion (after Jul 2003). <br /> A trace of liquid was found in the 91-and diesel-product submersible turbine pump (STP) sumps. A trace of oily <br /> debris was found in under dispenser containment(UDC) 7/8. Secondary containment shall be constructed to <br /> prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. The service technician <br /> removed the trace of liquid from the 91-and diesel-product STP sumps and the oily debris from UDC 7/8. <br /> This was corrected on site. <br /> This is a repeat violation, Class Il. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 9/21/2017. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
