Inadequate equipment maintenance <br /> • Non-calibrated instruments <br /> • Inappropriate sample containers <br /> ' Lack of Quality Assurance samples to assure precision of sampling methods and <br /> laboratory analysis <br /> • Sample disturbance during transportation <br /> ' Incorrectly labeled samples <br /> • Lack of or poor field notes <br /> ' NOA understands the effects of these common errors and will field activities in a manner to <br /> prevent these errors from occurring. <br /> 3.8 Sample Documentation <br /> 3.8.1 Sample Labeling <br /> ' Each sample will be marked with identification details, including the sample identification <br /> number, date and time of sample collection, sample matrix, whether the sample is preserved or <br /> unpreserved, signature of sampler, and the general analysis to be conducted. <br /> f3.8.2 Field Documentation <br /> A field logbook will be maintained and will describe the following: <br /> • Sample Matrix <br /> ' Sample Identification Numbers <br /> • Sample Collection Date/Time <br /> • Collection Method <br /> • Latitude and Longitude of sample as read from portable GPS unit <br /> • Reason for selecting the sample <br /> Description of any deviations <br /> • Other details as deemed appropriate by the person responsible for the data entry. <br /> ' The field logbook will be dated and signed by the NOA staff member collecting the samples. <br /> Alternatively, the field logbook will be dated and signed by the third party staff member <br /> collecting the samples. <br /> 3.8.3 Chain of Custody <br /> A chain of custody will be filled out and signed by the person responsible for the collection of <br /> samples, either NOA or a third party.
