INOA Drywell Closure and Investigation Page 11 of 26 <br /> May 30, 2014 <br /> 4.4.2 Field Documentation <br /> IA field logbook was maintained and described the following: <br /> • Sample Matrix <br /> • Sample Identification Numbers <br /> • Sample Collection Date/Time <br /> • Collection Method <br /> Latitude and Longitude of sample as read from portable GPS unit <br /> Reason for selecting the sample <br /> • Description of any deviations <br /> • Other details as deemed appropriate by the person responsible for the data entry. <br /> The field logbook was dated and signed by the NOA staff member collecting the samples. <br /> In addition, a Site Visit/Observations log was filled out for each drywell by the NOA staff <br /> member collecting samples. These logs are included as Plate 8 and Plate 9, Appendix A. <br /> 1 4.4.3 Chain of Custody <br /> A chain of custody was filled out and signed by the Geologist responsible for the collection of <br /> samples. The sample number, date and time of collection, location, sample type, preservative <br /> used (if any), analysis required, and the sampler's initials appeared on all chain of custody <br /> documents. <br /> 4.5 Sample Transportation <br /> 1 The sample containers were tightly sealed and labeled; the label showed the sample I.D. <br /> number and other pertinent information. The filled sample containers were placed on ice <br /> directly after collection. Samples were then picked up by the laboratory, McCampbell Analytical <br /> Inc. Samples were transported under chain of custody documentation to McCampbell Analytical, <br /> Inc. in Pittsburg, California by McCampbell staff same day as sample collection. Sample <br /> packaging, labeling and transportation followed the Department of Transportation regulations. <br /> 4.6 Equipment Decontamination <br /> All sampling equipment was decontaminated before and after each sampling event. All sampling <br /> equipment was disassembled, washed in a solution of Liqui-Nox detergent and potable water, <br /> rinsed with potable water and triple rinsed with distilled water. <br /> Cleaning was conducted in such a manner so as to contain the water, which was then placed in <br /> appropriate containment vessels, labeled, sealed, and stored prior to disposal. <br /> 1 <br /> 902 Industrial Way•Lodi, CA 95240•209.367.3701 •Fax 209.333.8303 02014 Neil 0.Anderson&Associates, Inc. <br />
