INOA Drywell Closure and Investigation Page 11 of 26 May 30, 2014 4.4.2 Field Documentation IA field logbook was maintained and described the following: • Sample Matrix • Sample Identification Numbers • Sample Collection Date/Time • Collection Method Latitude and Longitude of sample as read from portable GPS unit Reason for selecting the sample • Description of any deviations • Other details as deemed appropriate by the person responsible for the data entry. The field logbook was dated and signed by the NOA staff member collecting the samples. In addition, a Site Visit/Observations log was filled out for each drywell by the NOA staff member collecting samples. These logs are included as Plate 8 and Plate 9, Appendix A. 1 4.4.3 Chain of Custody A chain of custody was filled out and signed by the Geologist responsible for the collection of samples. The sample number, date and time of collection, location, sample type, preservative used (if any), analysis required, and the sampler's initials appeared on all chain of custody documents. 4.5 Sample Transportation 1 The sample containers were tightly sealed and labeled; the label showed the sample I.D. number and other pertinent information. The filled sample containers were placed on ice directly after collection. Samples were then picked up by the laboratory, McCampbell Analytical Inc. Samples were transported under chain of custody documentation to McCampbell Analytical, Inc. in Pittsburg, California by McCampbell staff same day as sample collection. Sample packaging, labeling and transportation followed the Department of Transportation regulations. 4.6 Equipment Decontamination All sampling equipment was decontaminated before and after each sampling event. All sampling equipment was disassembled, washed in a solution of Liqui-Nox detergent and potable water, rinsed with potable water and triple rinsed with distilled water.
