12/ 26/ 2001 12 : 57 209 - 579 - " 25 MODESTO ATC -� PAGE 05 <br /> SUPPLEMENTAL CONDITIONS <br /> FOR ENCROACHMENT PERMIT N0. 66695 <br /> The following are included as part of the conditions for approval of Encroachment Permit <br /> no. 66695 : <br /> 1 . Keep one lane of traffic open in each direction at all times. <br /> 2. All traffic control shall conform to Cal Trans Traffic Control Manual . <br /> 3 . No street closures are allowed with this permit. <br /> 4 . All work shall conform to the recently adopted Section 100 of the City of <br /> Stockton Standard Specifications, <br /> 5 . All lane closures shall include the use of a lighted arrow board . <br /> 64 All work in the paved area of Oak and San Joaquin Street's shall be <br /> contained within the parking lanes of those streets. The excavations shall <br /> be a minimum of five feet off of lip of gutter. <br /> 7. Any damaged sidewalk must be replace from score mark to score mark. <br /> 8 . The boring backfill material shall be a two (2) sack sand and cement slurry capped <br /> with thirteen (13) Inches of a six (6) sack cement mix, dyed with enough lamp black <br /> to appear homogenous with the surrounding asphalt. <br /> Failure to comply with the above conditions shall cause this permit to be immediately <br /> revoked.
