P Telephone:(209)468-3420 Fax:(209)464-0138 Web:www.sigov.org/ehd <br /> �Lhi <br /> Food Program Complaint Inspection Report <br /> Name of Facility: MAIN STREET DONUTS Date: 10/13/2017 <br /> Address: 1041 S MAIN ST, MANTECA 953375738 <br /> Owner/Operator: PRAK, RATHMONY Telephone: (209)823-0123 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00044725 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> C00044725 states: "COMPLAINANT ALLEGES THEY WERE AT FACILITY ON 10/10/2017 AT APPROXIMATELY 8:20 <br /> AM AND SAW A COCKROACH CRAWLING AROUND IN THE DONUT CASE." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility <br /> OBSERVATIONS:Observed several flies in the facility at the time of the inspection. Take action to reduce/eliminate flies in <br /> the facility. Correct today. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #44 Premises: Clean/Litter Free; Vermin-Proof <br /> OBSERVATIONS:Upon arrival, the front door of the facility was open. Operator closed door. Maintain all doors closed at all <br /> times to prevent the entrance of vectors. <br /> CALCODE DESCRIPTION: The premises of each food facility shall be kept clean and free of litter and rubbish all clean and soiled linen <br /> shall be properly stored non-food items shall be stored and displayed separate from food and food-contact surfaces the facility shall be <br /> kept vermin proof. (114067 6), 114123, 114143(a)&(b), 114256, 114256.1, 114256.2, 114256.4, 114257, 114257.1, 114259, 114259.2, <br /> 114259.3, 114279, 114281, 114282) <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Upon arrival, operator was not aware of the complaint. Operator states that the landlord sprays"every week"and they have <br /> not been experiencing a problem with cockroaches. <br /> Upon inspection, one dead cockroach was observed behind the 1 door prep unit(cleaned up on site)and one dead cockroach <br /> was observed on the dry goods storage shelf(cleaned up on site).
