PUBLIC REALTH SERV 1 E
C S
PpUlly
SAN JOAQUIN COUNTY
2 ';Z
ENVIRONMENTAL HEALTH DIVISION
Ernest M. Fujimoto, M. Box 388 a Stockton, CA 95201-0388 c4;FOR`P
209/468-3420
JIMMIE M RISHWAIN v
2410 BRITTANY CT
LODI CA 95242 MAILED NOV 0 81995
RE: FORMER GAS/SERVICE STATION
2130 Country Club Boulevard
Stockton, CA 95204
San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) is
currently requiring subsurface investigation at sites near this location. Ground water
analytical data collected just north of 2130 Country Club Boulevard indicates that petroleum
contamination is impacting the shallow groundwater. The monitoring well located just north
of your site is the "up-gradient" well for the former Chevron service station currently
undergoing their subsurface investigation.
In order to evaluate potential sources that may be impacting groundwater, PHS-EHD would
like to review any records you may have concerning any underground fuel storage tanks
previously located on your property. Of particular importance are records that indicate tank
installation dates, number of tanks, size of tanks, date of removal (if applicable), soil samples,
City of Stockton Fire Department removal permit(s), and owner/operator of the tanks, as well
as the history of the site's previous usage.
Sites that are suspect of having an unauthorized release may be required to investigate the
subsurface conditions in order to verify actual conditions. Prior to this, it is my hope that you
as the landowner, can provide to us the records that can support and verify that soil conditions
and site usage at 2130 Country Club are not a source that may have impacted the groundwater
now being detected immediately down-gradient from your site.
You may submit any correspondence or any questions you may have to Michael Infurna at
(209) 468-3454.
Donna Heran, REHS, Director
Environmental Health Division
""`:4_ 4
Michael Infurna, Senior REHS Diane M.
