TAILGATE SAFETY MEETING CHECKLIST <br /> Topics Covered <br /> (Check off as discussed) <br /> Personnel training/qualifications: Check cards for OSHA HAZWOPER 40-hour <br /> certification/8-hour-refresher training(other if appropriate). <br /> Supplies: Indicate location of first aid kit, fire extinguisher, clean water supply (drinking, eye <br /> wash), and Site Health and Safety Plan(SHSP). <br /> Emergency services: Discuss location of nearest telephone and directions to hospital. Map, <br /> directions, phone numbers provided at end of SHSP(Attachment Q. <br /> Site background: Discuss types, locations, and concentrations of chemicals found onsite, <br /> presence of free product, depth to groundwater, etc. <br /> Work activities: Discuss scope of work for the day and activities to be performed. <br /> Potential hazards: Discuss physical hazards (lifting, pinch points, traffic, working around <br /> machinery, etc.); chemical hazards (exposure limits, symptoms, air monitoring); and <br /> environmental hazards(heat stress, etc.). <br /> Air monitoring: Necessary equipment is onsite and calibrated. Circle: CGI PID <br /> Personal protective equipment (PPE): Discuss required level of protection. See that <br /> workers have appropriate PPE onsite; includes, but is not limited to, hardhat, steel-toe boots, <br /> safety glasses, ear plugs/ hearing protection, respirator(with cartridges), gloves, traffic safety <br /> vest(other ). <br /> Utilities: Utilities have been cleared/marked by appropriate divisions. <br /> Traffic control (vehicular and pedestrian): Work area is properly delineated and cordoned off <br /> from traffic. <br /> Compliance log: SHSP has been reviewed and signed by site personnel. <br />
