(ii) Operation of the injection facility is temporarily granted for the two <br /> (2) weeks following initial operations to allow for sample analyses to <br /> be performed and for the results to be submitted to EPA. <br /> (iii) The Permittee shall maintain copies (or originals) of all records <br /> relating to the hazardous waste determination and make such records <br /> available for inspection. <br /> (iv) The Permittee shall perform an additional hazardous waste <br /> determination whenever there is a process change or a change in fluid <br /> chemical constituents or characteristics. <br /> 2. Mechanical Integrity <br /> (a) Mechanical Integrity Tests "MITs") <br /> (i) Casing/tubing annular pressure internal MIT) <br /> A demonstration of the absence of significant leaks in the casing, <br /> tubing and/or liner hanger shall be made by performing a pressure test <br /> on the annular space between the tubing and long string casing. This <br /> test shall be for a minimum of thirty (30) minutes at a pressure equal <br /> to or greater than the maximum allowable injection pressure. A well <br /> passes the MIT if there is less than a five (5) percent change in <br /> pressure over the thirty (30) minute period. A pressure differential of <br /> at least 350 pounds per square inch ("psi") between the tubing and <br /> annular pressures shall be maintained throughout the MIT. <br /> (ii) Continuous pressure monitoring <br /> The tubing/casing annulus pressure and injection pressure shall be <br /> monitored and recorded continuously to an accuracy within one (1) <br /> psi. The average, maximum, and minimum monthly results shall be <br /> included in the quarterly report to EPA unless more detailed records <br /> are requested by EPA. <br /> (iii) Injection profile survey (external MIT) <br /> In conjunction with the initial FOT required in Section A paragraph <br /> 5(c), a demonstration that the injectate is confined to the proper zone <br /> shall be conducted and presented by the Permittee and subsequently <br /> approved by EPA.
