CONDITIONS OF APPROVAL <br /> MS-87-48 <br /> JOHN & CINDY SCOFIELD <br /> 1 . DEPARTMENT OF PUBLIC WORKS <br /> a . A parcel map is required . <br /> b . A 5-foot wide roadway dedication is required on Bender <br /> Road along the frontage of Parcel A. <br /> 2 . LOCAL HEALTH DISTRICT <br /> a. This tentative division of land shall meet the Rules <br /> and Regulations of San Joaquin County Ordinance X6549 . <br /> Prior to recordation of the final map , a percolation <br /> test and a minimum sewage disposal area are required <br /> for Local Health District approval on Parcel A . A <br /> permit for the percolation test from the Health <br /> District is required . <br /> b. The existing pit well on Parcel B must be destroyed or <br /> the casing extended 12 inches above grade and the <br /> excavation filled with earth or sand . The destruction <br /> or repair must be done under permit and inspection of <br /> the Local Health District within 30 days . <br /> 3 . PLANNING DIVISION <br /> a .
