Task 3 — Reporting
Upon completion of the field activities, a letter report for the abandonment of the well
will be prepared for client and agency review. The report will include:
• A description of the field activities and observations
• A California Department of Water Resources Well Drillers Report, DWR Form
188, REV 11-97 1987 .
All information gathered during the well abandonment activities by Kleinfelder is
considered confidential and will be released only upon written authorization from the
client or as required by law. California law requires a person to inform the state if a
situation is encountered that can be considered an immediate endangerment to the
public's health or welfare and/or to the environment. Therefore, the client will be
contractually bound to make any said disclosures in lieu of Kleinfelder, its owners,
principals, employees or subcontractors.
CLIENT RESPONSIBILITIES AND ASSUMPTIONS
• Adequate workspace and access will be provided. If the drilling equipment cannot
be positioned properly, other arrangements for the proper destruction of the wells
should be investigated.
• The client should be aware that penetrating the site's surface is inherently risky. It is
impossible to determine with certainty the precise location of all structures (such as
septic system tanks, leach fields, etc.), which may be buried in the ground.
Kleinfelder's fee is not adequate to compensate for both the performance of the
services and the assumption of risk of damage to such structures.
• The associated removed casing and cuttings from overdrilling will be placed in 55-
gallon drums and left on site. The disposal of the drums will be the responsibility of
the client.
• Underground Services Alert (USA) at 1-800-642-2444 provides a partial location
service free of charge for major utility lines. It is important that Balfour Beatty
Infrastructure, Inc understands that subsurface structures (such as septic systems,
leach fields, etc.) buried under private property will not be marked by the public
utility companies contacted by USA.
02001 PROP.ENV/ST07P230/FB.1v Page 3 of 5
©2007 Kleinfelder September 11, 2007
