Date: 01/07/2009 Subject Well Destruction Permits Dwg.No. Spec.Section 312319 SCOPE OF CHANGE: Change Order Request 28 involves obtaining destruction permits for wells OC2-23, and OC2-25 and destroying these wells as per San Joaquin County guidelines. Please perform the following work: 1. Obtain well destruction permits from San Joaquin County for piezometers OC2-23 (Station 881+20)and OC2-25 (Station 920+00). 2. Destroy piezometers OC2-23 and OC2-25 in accordance with the permit requirements. Piezometers OC2-23 and OC2-25 well permits(dated April 1997)could not be located. The remaining five well permits(B-62,B-63,B-64,B-68,& B-69)have been transferred to Sundt Construction and their destructions are part of the Contract. For the above requested work, Sundt Construction is authorized to proceed Time and Material basis as per General Conditions;Article 7 the cost of this work shall not exceed$10,000. If the cost to exceed this amount, please contact the Engineer prior to proceeding. Please keep records of all expenditures and submit daily extra work reports to District Inspector.A change order will be issued once proposed change order is received. Attachments: 1.
